Along with your registration form here you will find all the necessary student documents that must be reviewed and accepted for any event.
By clicking on the terms and conditions during your online registration you agree to the following forms on this page.
Day Camp Cancellations Made MORE than 4 weeks before camp (Full Refund/Transfer)
Cancellations must be made in writing (email esteamlearinglabs@gmail.com). Please indicate if you want a full refund or if you want to transfer to another camp. The entire amount paid – $25.00 (Admin Fee) will be refunded, or can be transferred to any available camp week(s) of your choosing.
Day Camp Cancellations Made LESS than 4 weeks before camp (Partial Refund/Transfer)
Cancellations must be made in writing by noon on the Friday (email esteamlearinglabs@gmail.com) 1 week before the start of the week you wish to cancel you will receive a 50% refund.
Day Camp Cancellations Made the week of camp (No Refund/ Transfer)
Cancellations and no-shows receive no refund or transfer.
Late Registration Admin Fee
Any day camp registration added after online registration closes will incur a $15 admin fee. Online registration closes at midnight on the Thursday before each camp week starts.
Cancellation Requests for All ESTEAM Learning Labs Programs
Cancellation requests must be submitted to our office in writing via email at esteamlearinglabs@gmail.com. Not attending the program does not entitle you to a refund. When you register your child, you are reserving space, time, and staffing whether or not your child attends.
I also understand that participation in any activities is entirely voluntary and requires participants to follow instructions and abide by all applicable rules and the standards of conduct.
Thus, all participants must understand and follow the camp guidelines and rules. These guidelines and expectations are in place to ensure the safety of your child and staff.
1. Listen to the staff.
2. Respect Day Camp staff and other camp participants.
3. Respect camp property and facility property.
4. Keep hands, feet, and other objects to yourself.
5. Participate in camp activities.
6. Use an inside voice when indoors.
7. Clean up after yourself.
8. Be positive and have fun!
Should a participant choose not to follow any of these rules, these are the guidelines that the staff
will follow to handle the situation:
Step 1: Verbal Warning.
Step 2: Reset (Time time away from the group)
Step 3: Email to Parent.
Step 4: Removal from the program
In the event that a Camp participant engages in behavior that poses a threat of bodily harm to himself, others, or facility property, an immediate meeting with the parent(s), or guardian may be called. If such behavior warrants it, immediate removal from the program will happen.
Situations that will result in an automatic Behavior Warning report are stealing, use of profanity, bullying, excessive violence (hitting, kicking, biting, etc.), and property damage.
Please review these rules with your child. The staff will review these rules with your child at the beginning of camp. Thank you in advance for your cooperation, and we hope to have a great summer!
In case of an emergency involving me or my child, I understand that efforts will be made to contact the individual listed as the emergency contact person by the medical provider and/or adult leader. In the event that this person cannot be reached, permission is hereby given to the medical provider selected by the adult leader in charge to secure proper treatment, including hospitalization, anesthesia, surgery, or injections of medication for me or my child. Medical providers are authorized to disclose protected health information to the adult in charge, camp medical staff, camp management, and/or any physician or health-care provider involved in providing medical care to the participant. Protected Health Information/ Confidential Health Information (PHI/CHI) under the Standards for Privacy of Individually Identifiable Health Information, 45 C.F.R. §§160.103, 164.501, etc. seq., as amended from time to time, includes examination findings, test results, and treatment provided for purposes of medical evaluation of the participant, follow-up and communication with the participant’s parents or guardian, and/or determination of the participant’s ability to continue in the program activities.
(If applicable) I have carefully considered the risk involved and hereby give my informed consent for my child to participate in all activities offered in the program. I further authorize the sharing of the information on the registration form with any ESTEAM Academy, ESTEAM learning Labs, volunteers or professionals who need to know of medical conditions that may require special consideration in conducting any activities.
With appreciation of the dangers and risks associated with programs and activities, on my own behalf and/or on behalf of my child, I hereby fully and completely release and waive any and all claims for personal injury, death, or loss that may arise against the ESTEAM Academy, ESTEAM learning Labs,, the activity coordinators, and all employees, volunteers, related parties, or other organizations associated with any program or activity.
NOTE: Due to the nature of programs and activities, ESTEAM Academy and ESTEAM learning Labs, cannot continually monitor compliance of program participants or any limitations imposed upon them by parents or medical providers. However, so that leaders can be as familiar as possible with any limitations, list any restrictions imposed on a child participant in connection with programs or activities an your registration form.
